Coordinated Care for Complex Needs
Our ECM program offers eligible members access to a dedicated Lead Care Manager. This manager plays a crucial role in overseeing and coordinating comprehensive healthcare services, ensuring seamless integration across various health and social care settings. Whether our members require assistance from doctors, dentists, social workers, or community centers, our ECM team is there to provide continuous support.
Connecting to Community Supports
Alongside our ECM services, Serve the People Community Health Center facilitates access to a range of Community Supports. These services are designed to address the social needs that impact health, such as housing security and access to medically tailored meals for recovery. Our goal is to empower our members to lead healthier lives and reduce the need for more intensive, costly medical interventions.
